Sending a Form
Once you’ve created your form in Form Builder, you're now able to send a Form. There's a couple of different options for this:
Sending a form to a Client
Sending a form to an Employee
Adding a form to a client record, for an internal procedure
To send a form to a client, go into a Client record.
Click Add New in the top-right hand corner
Select Form Request
On the next screen:
The client will automatically default to the client record you're on
You can personalise the invitation text to your client, this can be formatted and even include links if needed
Select the form you want to send to your client
Once you've selected the relevant form, click 'Send Request'
Clients must have write access on their client portal, in order to complete form requests
On the next screen, you can see the Form Submission Requests. Once a client has completed a form request, this will change from No to Yes
The client will receive a notification on their Client Portal dashboard, to inform them they have a form to complete
Once the client clicks View, they'll be taken to the form submission request screen. To edit the form, they need to click the pencil icon
Clients can then:
Update their details, as request in the form
Save Progress as they go
Submit the form, once completed
Once the form has been completed by the client, you will receive a notification in your own dashboard to confirm completion
You can then go in and view the submission and details of the form

Save Progress, as they go
Submit the form, once completed

To add an internal form, related to a client, go into a Client record.
Click Add New in the top-right hand corner
Select Form Request
On the next screen:
You can personalise the text to label the form request
Select the form you want to send to attach to the client record
Once you've selected the relevant form, click 'Send Request'
This then creates the form against the client record, but is not viewable by the client via their portal access.

To complete the form, go into the Form Submission Requests screen via the client record (as above) and click into the form you want to update
On the next screen, click the pencil to go in and edit the form
You can then:
Complete the relevant details
Save Progress, as you go
Submit, once completed
Via the Form Submission page, you can view the details that have been put into the system
